Registration Fee: A one time, non-refundable $150 enrollment registration fee will be charged to each new account. This applies to individual (s) or family. 

Membership Fee(s): The Town Doctor® membership fee(s) begins upon enrollment, and is non-refundable. All future membership fees are paid by the calendar month until notice of cancellation. Payment of membership fees must be maintained and is required to remain an active member and receive service.
